Description: For secondary students (with confirmed disability) in years 7-12 in Dept of Educ public schools (not Catholic or independent schools) in Northern Sydney region. They provide advice and support to facilitate the transition from school to community life, through Individual Transition Plan that may include various programs and resources; links parents of a child with a disability and the Department of Education, employment and support agencies and government departments in the disability field. Services could include day support services, education and training or employment support programs. SLEC or SCP provide funds for school leavers with high and moderate support needs, usually for up to two years. Support for many post school programs is part of the students NDIS plan if they are members of that scheme. Students transition to many settings from university, TAFE, Disability Employment Services, School Leavers Employment Support, Australian Disability Enterprises and day programs.
