Description: Multidisciplinary team providing intensive, short-term, home-based care to patients who require complex and/or technical care and/or access to a negotiated service, whilst recovering from an acute illness. A care plan is developed in consultation with the local General Medical Practitioner and Hospital Medical specialist and their team.
