Description: Assists youth aged 12-25 with chronic health problems move from paediatric to adult health services. The ACI Network Manager and three Transition Care Coordinators at St Leonards, Randwick, and John Hunter Hospitals extend the program across all Local Health Districts in NSW to provide information about adult health services, to assist teens and their family to find / attend / adjust to adult health services and staff (including a general practitioner). Helpful factsheets and publications on the website. The ACI team work closely with the Trapeze service which is the chronic care / transition service for the Sydney Children's Hospitals Network.

Hours: Mon-Fri 8:30am-5pm
