Description: This service assists people experiencing chronic homelessness and provides a vital connection point to access integrated support. Seeking out or responding to referrals for people sleeping rough by conducting assertive outreach, building meaningful connections, and finding opportunities for those individuals to become housed and connected to support beyond our care.
