Description: This service provider facilitates long term supported accommodation. It teaches and maintains independent living skills for residents in a group home or individual living environment. This provider caters for adults with an intellectual disability requiring low to moderate support. Interested persons should be engaged in day programs (either work or day programs). Also provides support to people in their private homes if they have appropriate funding. Also provides financial management, case management, fortnightly leisure activities and holiday planning to all residents on top of regular domestic assistance and personal care.

Hours: Mon-Fri 8am-5pm (Office Hours)

Disability facilities: not suitable for wheelchairs
