Description: Provides community-based palliative care to the Western Sydney region in partnership with the Western Sydney Local Health District (WSLHD) to support people with a life-limiting illness or complex needs so they can remain at home. Provides clinical care through a registered nurse service and palliative care doctors that work with clients’ medical specialists and general practitioners (GP) to ensure delivery of the highest standard of care possible. A team of social workers, spiritual care workers and volunteers can provide support on practical issues around finances, accommodation and social support, respite or counselling for carers, volunteers for additional in-home assistance and bereavement services.
