Description: Provides a range of 'Hospital in the Home' and 'Post Acute Care' services to children with a variety of acute illnesses and their families. Services may include assessment, ongoing interventions, education and support. CAPAC aims to prevent unnecessary admissions and facilitate the early discharge of children with acute illnesses. You will need a Drs referral from the hospital.
