Description: Australia's 31 Primary Health Networks (PHNs) are independent organisations working to streamline health services - particularly for those at risk of poor health outcomes - and to better coordinate care so people receive the right care, in the right place, at the right time. PHNs have 3 primary roles. They: - commission health services to meet the needs of people in their regions and address gaps in primary health care; - work closely with general practitioners (GPs) and other health professionals to build the capacity of the health workforce capacity to deliver high-quality care; - integrate health services at the local level to create a better experience for people, encourage better use of health resources, and eliminate service duplication. Use the locator map to find the location, boundaries and contact details of Primary Health Networks (PHNs).
