Description: This program supports young people with a disability to transition from the parental responsibility of the Minister for Community Services and to live as independently as possible. The program is designed for young people with a disability who will require a range of ongoing funded support from various providers, after 18 years of age. An important requirement is the provision of stable accommodation, with flexibility to meet changing needs.
