Description: Provides transitional nursing care service for frail aged and wheelchair users. Provides wheelchair accessible transport and trained professionals to accompany clients door to door and remain with them during their appointments. Provides individual care to enable clients to attend: specialist appointments, hospital pre-admissions, admissions and discharges, post-operative visits, dental appointments, weddings and social functions, funerals and community access outings. Additional services that can be provided are: verbal and written reports from Doctor to family and nursing home after appointments, food and clothes shopping, optometrist appointments, home visits, post-operative nursing care, 24/7 in home emergency assistance, regular community access outings and attending concerts.
