Description: In NSW the Mobility Parking Scheme (MPS) application form from Service NSW Centre or online. The MPS provides parking concessions to people with mobility disabilities. All applications for new or renewal MPS permits must be signed by a medical doctor. Proof of identity is required to ensure only eligible people receive a permit. The public can report abuse of the NSW parking scheme for people with disability by phoning 1300 884 899 or emailing info@service.nsw.gov.au. Callers to the hotline will be required to leave their name as well as the date, time and location of the alleged misuse of the MPS permit; a vehicle registration number and the MPS permit number. Also answer general enquiries.
