Description: Program aims to support people to establish and maintain meaningful social connection with community, leading to reduced social isolation and increased wellbeing. Social Connection Case Managers will assist in providing navigation and referral, to address individual social needs while building knowledge of social opportunities within the community. Community Connectors will provide direct opportunities for social connection, both one-on-one and in small group settings, fostering greater social confidence and a connection to community. Volunteer Community Companions will provide regular social support and companionship via telephone or video, increasing opportunities for particularly isolated individuals.
