Description: Provides social work, occupational therapy, NDIS and psychological support services in the community. PCCS are specialists in providing help to people living with long-term health conditions and mental ill health. Services also include: Social Prescribing and support groups for people with psycho-social needs; NDIS access, plan management, coordination of supports, and assessments by Occupational Therapists (OTs) or Nurse. Programs are free to access for eligible residents and include: "Head to Health" mental health support "Psychological Access Plus" "Back to Home Back to Life" support for patients transitioning from hospital to home "GP Social Work Connect"

Disability facilities: Flat or ramp access from street, plus disability parking and bathroom

Languages: Italian, German, Persian, Hindi, Tibetan, Lao, Thao, Urdu, Pushto, Mandarin, Cantonese
