Description: General aged care assessment for conditions such as dementia, physical frailty or disability which may at least in part be remediable, e.g. following stroke or orthopaedic procedure/injury. Rehabilitation is provided on an outpatient basis (inpatients treated in a separate unit). Services include physiotherapy, occupational therapy, speech pathology and social work. Referral is made to appropriate services, e.g. home nursing, community services, specific patient education programmes and dementia support (including home visiting and support to carers and sufferers).
