Description: Provides: Rehabilitation to people with serious injury, joint replacement, transplants and aged care rehabilitation. Palliative care to people with a life limiting illness no longer responding to curative treatment. Supportive care to people receiving chemotherapy or radiation therapy who are experiencing difficulty managing symptoms related to treatment. Community palliative care service to people in their own home.

Hours: 24 hours, 7 days per week

Disability facilities: Ramps, lifts, accessible bathrooms and garden
