Description: CHSP funded service provider, servicing people over the age of 65 years from East Fremantle and the surrounding suburbs. Services include: Social Support individual - in home support and accompanied activities. Social support group- in Centre activities and Community Access (including outings). Centre based Respite. In home flexible respite and community access.

Hours: Mon-Fri 8:30am-4:30pm
