Description: Community Vision is a provider of Aged Care, Disability and Family Day Care services across the Perth Metro area. Our organisation is here to provide you with your choice of services to help you continue living independently in your community and supporting your family's needs. Our trained staff will provide assistance to meet your Disability, Aged Care and Dementia Specific supports, all tailored to meet your individual requirements, as well as provide any information required to support your childcare needs. Contact our friendly Customer Service Team on 1800 968 754 who will be happy to assist you. NDIS registered: NDIS Assist Personal Activities, Transport, Nursing Care, Life Skills, Household Tasks, Community Participation, Group/Centre Activities, etc.
