Description: Interchange Australia has been providing disability and aged care services since 1986. Our services are designed to meet individual needs to enhance independence, health and well-being. Interchange Australia is an approved provider of Disability Supports and Services under the NDIS and also an approved provider of Aged Care Services and Supports under the Commonwealth Home Support Program (CHSP) and Home Care Packages (HCP) Program. Interchange Australia also provides Support Coordination and Consultation to individuals and/or their carers to navigate the aged and disability care systems and source individually tailored solutions to current and future care needs. We also provide plan management via our LePlan service Australia wide.
