Description: Provides Assertive Case management including a range of treatment options which are based on the Recovery concept and covers clients' mental and physical health, social, emotional, vocational, spiritual, sexual and leisure areas of life. Support is provided to people seriously affected by mental illness who are registered clients of Mental Health Teams at Auburn, Parramatta, Hills and Merrylands. The service is designed to meet the personal and developmental needs of consumers with severe psychiatric disabilities requiring complex and high-level services, who have not been sustained in the community through traditional case management practice. They are referred by the Area's Mental Health Teams, MH Intake team or Inpatient MH Units.

Hours: Mon-Fri 8:30am-5pm
