Description: Self-managed funding enables people with disability and their supporters to decide and control the funding of their short to long-term goals to promote quality of life and valued social inclusion through community-based activities that strengthen skills and abilities. Programs include Community Participation Program and Transition to Work (school leavers aged 18+), Life Choices (ages 26-54), and Active Ageing (ages 55-64). Provides information on these programs to individuals or at community meetings.
