Description: The Platform aims to provide practical and emotional support through a range of programs, including supported accommodation to young people (12-15 years and 16-18 years) who are homeless. Outreach support services to young people 12- 25 years. Also provides related support to young people aged 12- 25 years and where appropriate, their families, through a case management model of support.
